Documents Required for FSSAI Licence (Basic, State & Central)

Missing or unclear documents are the top reason FSSAI applications get queries. Confirm your licence category on FoSCoS first, then collect only the papers that apply to your Kind of Business.
Documents every applicant should prepare
- Photo ID and passport-size photograph of the authorised person
- Address proof of the responsible person (Aadhaar, PAN, Passport, or Voter ID)
- Proof of business premises (rent agreement, ownership deed, or accepted utility bill)
- List of food products / categories and activities on FoSCoS
- Partnership deed, LLP agreement, or Certificate of Incorporation when applicable
Basic Registration (Form A)
For eligible petty and small FBO categories within Basic limits. Typically needs ID, premises proof, product list, and Form A details. Layout plans and FSMS are usually not required at this tier, but confirm for your KoB on FoSCoS.
State Licence (Form B)
Common additions include kitchen or plant layout, FSMS plan, water test report for manufacturers, machinery/capacity details, and municipality or fire NOCs where your category requires them.
Central Licence (Form B, Central)
Required for many importers and notified categories regardless of turnover. Add import product details, IEC-related papers, label proofs, multi-premises declarations, and Central-specific FSMS documentation.
Restaurant and cloud kitchen
Delivery platforms expect your 14-digit FSSAI number, legal name, and kitchen address to match onboarding records. Keep premises proof and product categories accurate for aggregator verification.
Food manufacturers
Plan for layout/floor plan, water test from a NABL lab when applicable, FSMS plan, machinery list, and manufacturing capacity inputs used to determine star category on FoSCoS.
Importers
Central Licence paperwork usually includes imported product categories, source countries, label compliance proofs, and IEC. Customs clearance depends on valid FSSAI import authorisation.
Name and address must match
FoSCoS entries, rent agreements, bank KYC, and ID proofs should tell the same story. Mismatches trigger clarification letters and delay approval.
